fa بررسی تغییرات شمارش سلولهای خونی بیماران سرطانی مراجعه کننده به بخش رادیوتراپی بیمارستان توحید سنندج در سال 1393 تخصصي Special پژوهشي Research <font size="4"><b> چکیده</b> </font><div style="direction: rtl"><font size="4"><b>زمینه و هدف:</b> سرکوب مغز استخوان از عوارض تهدیدکننده حیات، ناشی از پرتودرمانی است که متعاقب ایجاد نوتروپنی یا لکوپنی، موجب نیاز به بستری یا مصرف آنتی بیوتیکها در بیماران می شود. اختلاف نظر زیادی در بررسی معمولی شمارش تام سلول های خونی در هنگام رادیوتراپی وجود دارد. در حالی که عده ای انجام هفتگی آن را لازم می دانند و عده ای انجام آن را غیر لازم می دانند.به دلیل اهمیت زیاد کاهش اجزاء شمارش خونی در نتایج درمانی، این مطالعه تاثیر پرتودرمانی بر تعداد سلول های خونی را ارزیابی می کند. </font></div><div style="direction: rtl"><font size="4"><b>روش بررسی:</b> این مطالعه به صورت مطالعه توصیفی گذشته نگر قبل و بعد انجام شد متوسط سن بیماران 56 سال که با میانگین دوز 52 گری در 28 فرکشن مورد درمان قرار گرفته بودند. 170 بیماری که تحت پرتودرمانی قرار گرفته بودند با مقایسه میانگین شمارش سلول های خونی قبل، بعد و حین درمان مورد ارزیابی قرار گرفتند. منظور میانگین هموگلوبین، پلاکت، نوتروفیل، لنفوسیت و گلبول سفید ثبت و آنالیز شد. </font></div><div style="direction: rtl"><font size="4">یافته ها: بین سن با کاهش گلبول های سفید، هموگلوبین، لنفوسیت، نوتروفیل و پلاکت رابطه معناداری وجود داشت. (p<0/05). همچنین بین دوز کلی و تعداد فرکشن ها، هموگلوبین و تعداد پلاکتها با طول دوره درمانی و بین نوتروفیل با دوز کلی و تعداد فرکشن ها و طول دوره درمان رابطه معنی داری وجود داشت (p<0/05). کاهش سلول های خونی و نیاز به تزریق خون و آمپول PDG در بیماران با میانگین سن بالا بیشتر بود.کاهش گلبول های سفید در بیماران مورد بررسی رابطه مستقیمی با دوز کلی درمان و تعداد فرکشن ها داشت، کاهش میزان هموگلوبین و پلاکت خون بیشتر مرتبط با طول دوره درمانی بیماران بود.و میزان نوتروفیل در این بیماران با هر سه مورد ذکز شده مرتبط بود. شیمی درمانی همزمان یا قبلی و جنسیت هیچ رابطه معنی داری با تغییرات سطح سلولهای خونی نداشت. </font></div><div style="direction: rtl"><font size="4">نتیجه‌گیری: می توان نتیجه گرفت که پرتو درمانی منجر به کاهش سلول های خونی می شود که میزان این کاهش بسته به عواملی چون سن، طول دوره درمان، تعداد فرکشن ها و دوز کلی متغییر است. </font></div> <font size="3" face="times new roman, times, serif"><b> Abstract</b></font><div><font size="3" face="times new roman, times, serif"><b>Background and Aim:</b> Bone marrow suppression as a serious complication of radiotherapy induces leucopenia or neutropenia that results to hospitalization and antibiotic therapy. There is considerable controversy in scientific literature regarding the need to monitor the complete blood count of patients undergoing radiation therapy. While some advocate weekly examination of the peripheral blood, others deem it unnecessary. Since reduction in the number of blood cells has considerable effect on the treatment outcome, this study evaluates the effect of radiation therapy on the number of blood cells.</font></div><div><font size="3" face="times new roman, times, serif"><b>Methods and Materials: </b>This study designed as a retrospective descriptive study of pre- and post-treatment evaluation. The mean ages of patients was 56 years, and with a mean dose of 52 Gy in 28 fractions were treated. 170 patients who had received radiation therapy, evaluated by comparing the mean count of the circulating blood cells before, during and after therapy. For this purpose the average of hemoglobin, Platelets, Lymphocytes, Neutrophils and white blood cell count were recorded and analyzed. </font></div><div><font size="3" face="times new roman, times, serif"><b>Results:</b> there was a significant relationship Between age and the reduction in white blood cells, hemoglobin, lymphocytes, neutrophils and platelets (p<0/05). also, there was a significant relationship between white blood cells with total dose and number of fractions ,platelets and hemoglobin with duration of treatment and neutrophils with total dose ,number of fractions and duration of treatment(p<0/05). Reduction of blood cells and the need for blood transfusions and injections of PDG was higher in patients with a mean high age. Decline in WBC in the cases had direct relation with total dose and number of fractions.
