<p dir="RTL"><strong>زمینه</strong> <strong>و</strong> <strong>هدف</strong><strong><em>:</em></strong> مقاومت روز افزون باکتر­ی­ها به عوامل ضد میکروبی مشکل عمده در سراسر جهان می­باشد هدف از این مطالعه شناسایی عوامل باکتریال در عفونت­های ادراری و تعیین مقاومت آنتی­بیوتیکی آن­ها در افراد بستری و مراجعه کننده به بیمارستان توحید شهر سنندج در سال های 93-1392می باشد<span dir="LTR">.</span></p>
<p dir="RTL"><strong>روش بررسی</strong><strong>:</strong> در این بررسی 2406 نمونه ادرار به روش <span dir="LTR">Midstream clean catch</span> جمع آوری و سپس در دو محیط بلاد آگار و ائوزین متیلن بلو آگار با استفاده از لوپ استاندارد کشت داده شد . سپس برای 24 ساعت در دمای 37 درجه سانتی گراد انکوبه گردید <span dir="LTR">.</span> شناسایی ایزوله­های باکتریایی به وسیله روش های آزمایشگاهی و آزمون حساسیت میکروبی به روش دیسک دیفیوژن با توجه به استاندارد <em><span dir="LTR">Clinical and Laboratory Standards Institute</span></em> <span dir="LTR">(<em>CLSI</em></span>) انجام شدند. برای تجزیه و تحلیل داده­ها از نرم افزار<span dir="LTR">Excel 2013</span> و <span dir="LTR">SPSS V22</span> و آزمون کای دو و فیشر استفاده شد (05/0 <span dir="LTR">p<</span>).</p>
<p dir="RTL"><strong>یافته ها:</strong> از 2406 نمونه کشت ادراری مثبت، 5/65 % متعلق به جنس مونث و 5/34 % متعلق به جنس مذکر بود و با در نظر گرفتن 004/0<span dir="LTR">p<</span> ، این تفاوت معنی دار بود. شایع­ترین پاتوژن جدا شده <em>اشریشیا­کلی</em> و عامل 09/63 % از کشت­های مثبت بود. بیشترین میزان مقاومت پاتوژن های ادراری نسبت به آمپی­سیلین ( 15/64 %) و تری­­­­متوپریم­-سولفومتوکسازول (67/62 %). کمترین میزان مقاومت مربوط به ایمی­پنم ( 7/0 %) و آمیکاسین ( 01/1 %) بود. <em>اشریشیا­کلی</em> به عنوان شایع­ترین پاتوژن عفونت ادراری، بیشترین مقاومت را نسبت به آمپی سیلین (87/43 %) و کمترین مقاومت را نسبت به نیتروفورانتوئین ( 62/3 %) نشان داد.</p>
<p dir="RTL"><strong>نتیجه گیری:</strong> در مواردی که به نتیجه کشت ادرار دسترسی نباشد و یا نمی­توان منتظر جواب تست حساسیت آنتی بیوتیکی شد، آنتی­بیوتیک­های ایمی­پنم و آمیکاسین ممکن است در شهر سنندج بهترین انتخاب برای شروع درمان می­باشند.</p>
<p><strong>Background and Aim:</strong> Increasing resistance of bacteria to antimicrobial agents is a major problem around the world. The aim of this study was to indentify the bacterial agents in urinary tract infections and their antibiotic resistance determination in hospitalized and referred individuals to Sanandaj`s Tohid Hospital city during 2013-2014.</p>
<p><strong>Materials and Methods: </strong>In this study 2406 urine samples were collected with midstream clean catch method and cultured in blood agar and Eosin-methylene blue medium using standard loop. Then incubated for 24 hsr in 37 <sup>o</sup>C. Identification of bacterial isolates by laboratory method and antibiotic susceptibility test were performed by disk diffusion method according to <em>Clinical and Laboratory Standards Institute</em> (<em>CLSI</em>). For data analysis software Excel 2013 and SPSS V22 software and Fisher Exact test were used (p<0.05).</p>
<p> <strong>Results:</strong> Of 2406 urine samples positive culture, 65.5% belongs to females and 34.5% belongs to males and with taking p<0.004, this difference was significant. The most common isolated pathogen was <em>Escherichia coli</em> and cause of 63.09% positive cultures. Most of urinary pathogens resistant were to ampicillin (64.15%)<span dir="RTL">,</span> trimetoprim -sulfamethoxazole (62.67%). The least rate of resistance was to imipenem (0.7%) and amikacin (1.01%). <em>E. coli</em> as most common pathogen of urinary tract infections showed the most resistance to ampicillin (43.87%) and the least resistance to nitrofurantoin (3.62%) indicated.</p>
<p><strong>Conclusion: </strong>When the urine culture is not available or it is not possible to wait for the antibiotic susceptibility test, antibiotics imipenem and amikacin may be the best choice to begin the treatment in the Sanandaj city.</p>
